Sample Workshop Evaluation Questionnaire
Questionnaires are undoubtedly the single most frequently used type of evaluation instrument. While poorly designed questionnaires -- a "smile-o-meter" or "happiness indicator" -- are often administered at the close of a workshop, a well-designed instrument, such as the one shown in the Sample Evaluation Questionnaire, can provide a wealth of information.
Instructions
Questionnaires can take many forms, such as checklists, rating scales, multiple-choice questions, or open-ended questions. Most instruments include a combination of several different types of questions or items; aim for a variety of quantifiable data and subjective comments. Try to design your evaluation questionnaire carefully so that you get the information you need without requiring the persons completing the questionnaire to spend too much of their time.
